Health & Safety - Required Immunizations
• Documentation of Student Immunization Status MUST be Submitted to School Authorities BEFORE Any Child Will be Permitted to Attend School
| Required Immunization | Explanation |
Diphtheria/Pertussis/Tetanus |
A minimum of four (4) doses of DPT are required. One (1) dose must have been administered on or after the fourth birthday. |
Polio |
A minimum of three (3) doses of Oral Polio Vaccine are required. One (1) dose must have been administered on or after the fourth birthday. |
Measles |
Two (2) doses of Measles Vaccine are required. To meet this requirement both doses must be administered on or after the first birthday with a minimum of thirty (30) days between vaccinations. Vaccines received before the first birthday do not qualify as one of the two required doses. |
Mumps |
One (1) dose of Mumps Vaccine administered on or after the first birthday. |
Rubella |
One (1) dose of Rubella Vaccine administered on or after the first birthday. |
Tuberculin Testing |
Anyone who is transferring in from another state or country must be tested using the Mantoux intradermal test using 5 TU of stabilized PPD Tuberculin. This is the only T.B. test accepted and will only be considered valid if it was administered within the previous six (6) months. |
Hepatitis B |
Kindergarten/Grade 1 Students - Every child entering kindergarten or 1st grade (or a comparable age level special education program), whichever is first, must have received three (3) doses of the Hepatitis B Vaccine.
Grades 5,4,3,and 2 Students - When entering 6th grade, all students must have received three (3) doses of the Hepatitis B Vaccine. This series takes six months to complete.
Grade 6 Students - Every child entering grade 6 in September 2002 (or a comparable age level special education program), must have received three (3) doses of the Hepatitis B Vaccine.
September 1, 2003 - All Students entering grades 9 through 12 must have the hepatitis immunization series |
Haemophilus |
One (1) dose of Haemophilus Influenzae B (HIB) is required between the ages of 12 months and 59 months of age. |
Varicella |
One (1) dose on or after the first birthday or proof of disease. |
Tdap Booster |
Child born on or after January 1, 1997, AND entering or attending 6th grade on or after September 2008 must receive one dose of Tdap vaccine. One dose of Tdap shall be given no earlier than the 10th birthday |
Meningococcal |
Child born on or after January 1, 1997, AND entering or attending 6th grade on ar after September 1, 2008 must receive one dose of meningococcal vaccine such as the medically preferred meningococcal conjugate vaccine (MCV) |
Influenza |
Child between the ages of 6 months and 59 months and attending a day care center or preschool shall annually receive at least one dose of influenza between September 1st and December 31st of each year |
Pneumococcal |
Every child 2 to 11 months of age and enrolled in any child care or preschool facility shall receive a minimum of two age-appropriate doses.
Every child 12-59 months of age enrolled in or attending a child care center shall have received at least one dose of Pneumococcal Vaccine on or after their first birthday. |
